Cause

This occurs because the SFW vxpal component creates access handles on the gatekeeper devices that remain open forever.
Therefore when the storage is reconnected, the disks remain unreadable as the previous stale handles are not closed.

Resolution

Apply Storage Foundation for Windows 5.1 SP2 CP3 to fix this issue.

Issue/Introduction

Loss of all storage paths on EMC CX300 with EMC PowerPath 5.5. When reconnect all
storage paths back to host server, disk will not come back until recycle vxvm
service or reboot the system. This issue only happens when system installed with both Storage Foundation for Windows 5.1 SP2 and EMC PowerPath 5.5 on Windows server 2008 X64 SP2.
