This message is logged by Volume Manager when one of its ioctls fails against a disk device, typically when a disk is initialized or when vxconfigd starts.

Example message:

The issue can occur when the EMC BCV device sare in the ESTABLISHED state, or if the  EMC SYMMETRIX luns have hadwrite_disable attribute set via SYMCLI.

The message indicates that vxconfigd has attempted to execute an ioctl against a device file, but the device is not available. Error number 6 states that the operations failed due to "no such device or address".
