Issue

When disks in Veritas Storage Foundation are configured with MPIO, several events for device removal get logged.

Error Message

V-218-40960-12 - Device (%2,%3,%4,%5) is removed from array %6 of %7

 

Resolution

  • Check to see if any HBA failure.  Ignore if there is no failure and number of DSM path(s) is/are shown correctly. (sometimes, during system start up, system can initialize the disk, so device can go and come back to the system)
  • Check for vxdmpadm logs for any errors regarding path failures
  • Make sure the HBA drivers, storport/miniport drivers and MPIO drivers are updated. If using Veritas DMP/DSM ensure to have the latest DDI package installed
