Resolution
If the patch 125731-02 or later (125731-04 at time of this article) is not installed when the RP1 patches are installed with patchadd the volume manager patch 139352-02 will not be installed. All other patches to VCS, VxFS, etc. will be installed. The error is shown on the patchadd output, however it is not prominent and is and has been easily missed.
The missing patch 125731-02 is a XML and XSLT libraries OS patch that volume manager needs. It has no dependencies and installs easily.
The pkginfo -l output on volume manager will be in the PSTAMP field showing as RP1 installed yet the actual vxio module will not be the RP1 because the binaries have not been replaced. The solution is to install the 125731-02 (04) and then use patchadd to add the RP1 again. This will install the missing 139352-02 patch and correct the partially installed status.
