Cause
Following messages are seen in the syslog after executing the fsck command:

fsck is failing due to disk i/O error on c4t0d7 on which this filesystem's VxVM Volume is created.
Resolution
Check with the Storage/Hardware/OS vendor on the hardware I/O errors. If disk is faulty, it may require replacement and restoration of filesystem data from backups if there are no other existing mirrored plexes for the volume.
